Follicular Unit Extraction: The Best Treatment for Male Pattern Baldness

Follicular unit extraction (FUE) is a good alternative to conventional hair strip transplantation for most people with advanced hair loss. Rather than removing a complete section of scalp, FUE involves harvesting and transplanting individual follicular units, usually from the back of the head, to cover bald patches. Once in place, these units function exactly like any other healthy hair follicle, continually producing new hair. Furthermore, because hair from the back of the head is genetically “programmed” to remain active, transplanted hairs are highly resistant to hair loss.

Unlike the strip method, FUE will not create a visible scar, so it’s ideal for patients with hair too sparse to hide scar tissue. How Does PRP Work to Restore Hair?

Platelet-rich plasma (PRP), is concentrated blood plasma which contains a much heavier concentration of platelets than normal (about five times as much). Platelets are blood cells which aid the healing of wounds by promoting the blood to clot. PRP has been an active ingredient in medicines for the last twenty years.

The potential which PRP has for treating hair loss is revolutionary and has become of keen interest in aesthetic medicine.

The injection of PRP is a procedure which uses the body’s blood to generate healing. Doctors may also incorporate its use as a procedure for thinning hair, to transplanted hair, or to promote healing. There have been many patients who have benefitted from this procedure.

PRP and Hair Loss

When hair follicles or hair roots are healthy so is hair growth. These hair follicles’ health depends on the nutrition they receive from a healthy blood supply. By introducing Platelet-rich plasma where hair follicles are damaged or unhealthy, it enhances the ability for the body to heal these follicles naturally. Many doctors believe PRP can stimulate the growth of hair follicles and prevent hair loss.

What to Expect

PRP is a three stage process.

The first stage is to extract blood from your body and prepare it for injection. This process is very similar to having a routine blood test. A nurse will take your blood and then place it in a centrifuge to separate its components. The method will produce a concentrated amount of platelet-rich plasma which is ready to be applied to the scalp.

Next, the scalp needs to be stimulated to activate a wound healing process. Before stimulation, a topical anesthesia is applied to the scalp where the treatment will be administered. A micro-needling roller device is passed across the areas to be treated to induce the healing process. This microneedle uses 192 titanium needles which pierce the skin and open tiny wounds. Once the wounds have been created, the highly concentrated PRP is injected into the scalp immediately. A special dressing is then applied to the scalp, and photodynamic treatment is administered to the injected area to aid the effects of PRP. There will be mild discomfort during the treatment, but you can return to normal activities almost immediately. The scalp will have to be protected from the sun.

Suitable Candidates

Not everyone is a suitable candidate for PRP treatment. Patients who have the following issues are not suitable candidates.

The entire procedure lasts from one to two hours depending on the quantity of hair loss. How Does FUE Work to Restore Your Hair?

Follicular Unit Extraction (FUE) is becoming a preferred hair restoration procedure by men and women alike. This relatively new method of harvesting donor grafts is less invasive than previous procedures and is a well-established hair replacement treatment. However, outcomes can differ drastically due to the skill level of the health care provider who administers the treatment. At Ritacca, we have developed a highly advanced FUE procedure which produces optimal results and healing.

The FUE Process

Before follicular unit extraction, a large section of the donor zone must be shaved to permit the collecting of a suitable number of grafts. When performing small FUE procedures, widespread shaving is unnecessary. FUE grafts can be harvested without cutting or shaving the hair at all.

Patients receive a local anesthesia for the procedure, and under a high-powered magnification, your doctor will select and extract individual follicle units. For removal, a tiny 0.75 to 1.00 mm punch is used to cut around the follicle unit. This procedure is minimally invasive because it requires cutting only a superficial layer of skin with tiny holes. Follicular unit extraction seems best suited for patients who will continue to wear their hair shaved, or who want to avoid surgery or discomfort.

Receptor Sites

The receptor sites are where your doctor performs incisions for follicle unit graft placements. This part of the procedure is the artistic portion because of the preference of sites chosen. Also, the choice of cut-to-size micro blades can make this procedure anything but “cookie cutter” in practice.

The blades can range from .7 mm to 1.3 mm in width and are fashioned for each patient, based on exact individual measures. The size of the patient’s grafts, the features of the patient’s skin, and the mass at which the grafts will be inserted all govern the creation of the receptor blades. This process allows the doctor to perform a transplant at densities of 50 grafts per square centimeter. Design distinctions such as direction and angle of hair growth, hairline shape, crown spiral, and hair flow are obtained by subtly changing the combination and orientation of these tiny incisions. We believe a denser and more natural outcome is attained through this process.

Graft Placement

Once your doctor has finalized his design, a team of nurses or certified surgical technologists will insert the grafts into the incisions. The grafts are placed using very fine forceps. The placement of follicles may take a few hours but is painless and reading or watching TV can pass the time.

The grafts are stored in saline under the ideal temperature conditions during placement. They are handled with extreme care to guarantee survival. Once the grafts have been inserted, your doctor will inspect the area to ensure the grafts are situated correctly. A blow dryer is used blow cool air on the recent grafts to help form a natural adhesion and set the grafts in proper placement. Instead of bandages, the patient will wear a baseball cap immediately after the procedure.
